Damaged Records

Records, including books, papers, maps, photographs, or other documentary materials are vital to the business of the government. In the event that records held by a Federal agency are removed, defaced, altered or destroyed in the wake of a disaster, the affected agency should promptly notify the National Archives and Records Administration (NARA) concerning those compromised records. The form below should be used to report to NARA the destruction of federal records.
