SF-8 Unemployment Compensation Notice

If you plan to file for unemployment compensation, download and print this form. Take this form along with your furlough notice and the other documents described on the form to your local public employment service office.

If you file for unemployment benefits, state unemployment offices are accustomed to having an SF50 Personnel Action to help adjudicate your claim. Federal agencies, following guidance from the Office of Personnel Management, are not issuing SF50s at this time. The Department of Labor advises that in addition to the SF8, you should provide the state unemployment office with a copy of your most recent Leave and Earnings statement. Your Leave and Earnings statement includes the monetary information state offices can use to determine the amount of unemployment benefits you may be eligible to receive. Department of Labor also recommends you provide a copy of your most recent W-2 would work as well. Both your Leave and Earning Statement and your W-2 can be downloaded from Employee Express. Please note that if you receive back-payment of wages after the shutdown and you have received unemployment benefits, you will be required to repay your unemployment benefits.
