U.S. Government Forms (Standard and Optional)
The General Services Administration (GSA) maintains a Forms Library containing hundreds of electronic forms in a variety of formats including Accessible FormNet, PDF, and Form Flow. GSA's Forms and Printing Policy Division, Office of Government wide Policy, has responsibility for obtaining required approval for these forms. The Library includes the following forms:

  • Standard Forms (SF) are required forms to be used by all agencies.
  • Optional Forms (OF) are made available for use by all agencies but are not required.
  • GSA Forms are those originating within GSA. They are most often used by GSA associates, contractors and customers.
