This certificate is awarded to the individual listed below for successful completion of the FEDERAL PROGRAM (FEDP) - GOVERNMENT CERTIFIED DRUG TEST SPECIMEN COLLECTION TRAINING, required for conducting DOT Drug Test Specimen Collections in accordance with 49 CFR Part 40.
The training included the study and review of the DOT Urine Specimen Collection Procedures, the Urine Specimen Collection Handbook for the New 5 - Part Federal Drug Testing Custody and Control Form, and performing 5 mock error-free collections. A 50 question test concerning the requirements of 49 CFR Part 40 was also successfully completed.
