DOI INHOUSE SPECIMEN COLLECTION PROCEDURES (PICTORIAL)

For 7 Part Federal Drug Testing Custody and Control Form (CC Form)


(collections performed in accordance with HHS and DOT regulations)

1. DONOR provides specimen at local clinic, hospital, doctor office, or onsite facility.

2. DONOR must provide valid photo identification.

3. DONOR provides specimen - UNOBSERVED.

4. COLLECTOR ensures specimen in 90-100 degree temperature range; AND ensures 30 or 45 ml of urine provided (single or split).

5. COLLECTOR completes Step 1 and 2 of the Federal Drug Testing Custody and Control Form (CC Form).

6. Example of completed CC Form, Steps 1 and 2.

7. COLLECTOR places barcode label(s) and specimen bottle custody seal(s) on bottle(s); AND records date(s) on seal(s).

8.Example of specimen bottle custody seal(s).

9. COLLECTOR dates; AND DONOR initials specimen bottle custody seal(s).

10. DONOR completes Step 4 of the CC Form.

11. Example of completed CC Form, Step 4.

12. COLLECTOR completes Step 5 and 6 of the CC Form.

13. Example of completed CC Form, Steps 5 & 6.

14. COLLECTOR places sealed specimen(s) in plastic tamperproof bag; AND seals bag.

15. COLLECTOR places specimen(s) in bag(s) into shipping box(es).

16. COLLECTOR places Copy 1 and 2 of CC Form in box(es).

17. COLLECTOR closes; AND seals shipping box(es) using the specimen bag shipping custody seal(s); AND initials and dates the box(es) seal(s).

18. COLLECTOR provides Copy 5 of CC Form to DONOR.

19. COLLECTOR completes Federal Express Airbill for shipment of specimen to SAMHSA/NIDA certified lab.
